Failure to identify, understand, or realize your risk does not alleviate it nor cause it to disappear. Do *not* assume that the limits of your knowledge and capability apply to everyone. Do *not* assume that since some particular problem you are aware of has yet to have occurred means it can’t occur. Both fall well within the realm of wishful thinking.

There used to be a column in one of the computer magazines called “Managment Speak Translations” or some such. Your post reminds me of one of those translations I clipped, and have hanging over my desk:
MANAGEMENT SPEAK: I think what we need is a more strategic plan. We need something more high level
TRANSLATION: I don’t understand the details, so lets pretend they aren’t important.
Hmmm I have recent experience with exactly this behaviour. 🙂
